Terms and Conditions for Zen Art and Wellness, LLC

The terms and conditions outline the rules and regulations for the use of Zen Art and Wellness Website, located at www.zenartandwellness.com. By accessing this website we assume you accept these terms and conditions. Do not continue to use Zen Art and Wellness, if you do not agree to take all the terms and conditions stated on this page.

We employ the use of cookies, By accessing your piece, you agreed to use cookies in agreement with the Zen Art and Wellness LLC Privacy Policy. Most interactive websites use cookies to let us retrieve the user’s details for each visit. Cookies are used by our website to enable the functionality of certain areas to make it easier for people visiting our website. Some of our affiliate/advertising partners may also use cookies.

Agreement to Provide Art Therapy as an Art as Therapy Approach

The client, you, are acknowledging the acceptance of using tele-therapy as a services for art as therapy sessions and/or counseling sessions. The therapist and the client, you, are located in separate locations. A platform is used for secure sessions, and the client, you, will be provided the link for tele-therapy sessions. The therapist will provide that source of platform for the tele-therapy sessions when you book your appointment/at the time of your appointment. In the event there is a disruption to the service, both the therapist, and the client, you, will try to reconnect again via internet on the platform. In case that does not resolve the issue, an email will be sent by the therapist to reschedule the session, if the disruptions are longer than 20 minutes. the rescheduled appointment will be rescheduled in a one-week time period only, if the disruptions are from internet connections.

The client, you, will need access to internet connection and digital form to be able to connect to the platform, via laptop, computer, tablet or phone that has capabilities for internet connection and microphone.

The client, you, will need to provide adequate microphone in order for the sessions to occur, as there will be music and/or communication for your session. The video and microphone must stay on at all times during the session.

The client, you, can sign up for sessions by self-pay, no insurance will be accepted. There will not be a refund for the sessions. Only if the sessions had internet disruption lasting more than 20 minutes will be rescheduled at no cost to the client, you, in a one-week time period.

The client, you, as the recipient will have to create an appropriate space for tele-therapy sessions. You will be responsible to ensure privacy when you are in session with the therapist.

The therapist will also have a private and secure location when connected to you, the client.

The client, you, will be responsible to have the art supplies, if you are booking an art therapy session, suggested by the art therapist, a list of supplies is located under art supplies on the website, www.zenartandwellness.com

If the client, you, is unable to have the appropriate supplies, you will need to let the art therapist know what supplies you have at the time of the session, for the art therapist to be able to adapt the session to the supplies.

If the client, you are booking art therapy sessions only, if you have a mental health concern, you will be referred to see a mental health counselor in your community. If the art therapist recommends for you to seeks mental health counseling, referrals will be suggested. If you already have a counselor, your diagnosis would be noted in the record keeping.

Music may be played during the art therapy session only, by the facilitation of the art therapist when appropriate for the art making process.

Agreement to Provide Counseling Services

Counseling will be held using an online secure HIPPA compliant video site, which the therapist will email you the link.

The client will have a secure and private area to have their online sessions. Client will have sign the necessary forms for treatment and will pay at the time of the session.

A mental health diagnosis will be made and a treatment plan will be written for counseling sessions.

Risks for tele-therapy sessions

There may be technical failures with internet connections

There may be technical issues with microphone.

Tele-therapy sessions may introduce privacy risks, please ensure you are in a safe and private location for the sessions.

Adolescent Services

Zen Art and Wellness, LLC, provides both counseling and art as therapy for adolescences age 13-18 years old. They will need to have appropriate behaviors for art supplies and be able to use the online video appropriately. At the time of the session, the clients, guardian/parent must allow client to have privacy during the session. Consent forms must be completed prior to the session, as the therapist will send them to you either by email or by the documentation platform.

Artwork online

Any artwork posted online will be the possession of Zen Art and Wellness, LLC. If artwork made by the client, you, is posed on the website or other social media sites by Zen Art and Wellness, LLC, a release will be sent to the client, you, to sign, to give Zen Art and Wellness, LLC, the ability to use for purposes of the website/business/social media. The client, you, may not republish material from Zen Art and Wellness, LLC.

Website

Information on the website and other social media by Zen Art and Wellness LLC, is the possession of Zen Art and Wellness LLC. The client, you, agree to not use any information, photos, or images or graphics as your own. The client, you, may not republish material from Zen Art and Wellness, LLC.

The client, you, have access to email the counselor/art therapist, at zenartandwellness@yahoo.com. Allow up to 24-hours for art therapist to respond.

Record Keeping

There will be record keeping in a secure HIPPA compliant platform.

Pricing

Pricing is subject to change, by the owner, Caroline Coreno, ATR, Registered Art Therapist, Zen Art and Wellness LLC, reserves the right to make changes to the pricing, when necessary at any given notice.

The client, you, will be paying for the session out of pocket, no insurance will be taken. The sessions will be paid in full; no refund will be made.

Payment will be made through an online portal. It is secure for the transaction.

If the session had internet disruptions, that lasted for more than 20 minutes, the the session will be rescheduled at no cost to you, the client, within a one-week period to reschedule the disrupted session.

A 24-hour notice of cancellation must be made to Zen Art and Wellness LLC, through the email, zenartandwellness@yahoo.com. If you do not give a 24-hour cancellation, then you will be charged $60 for it.

No Show Appointments will be charged $60.00

Disclaimer

The practice is in the state of Ohio. The art therapist and counselor is licensed for counseling in Ohio only. Therefore can only accept Ohio residents. Zen Art and Wellness LLC, is a counseling service and provides art as therapy sessions. There will be a diagnosis for counseling services. Recording keeping will be for all services.

Ethical Principles for Art Therapist, American Art Therapy Association, INC

May be requested. Zen Art and Wellness, LLC will give you the link to the website of the code of ethics for art therapist, from the American Art Therapy Association. Or Zen Art and wellness will email you the document of the code of ethics.
